Reconstruction following nipple-sparing mastectomies of large ptotic breasts remains a challenging endeavor. However, there exists several techniques in the plastic surgeon's armamentarium to preserve the native nipple. The current case will discuss proper patient selection and available options for immediate and delayed reconstruction following nipple-sparing mastectomy.
