There are four cardinal compartments for fat augmentation. The two deep are the nasolibial fold, the deep malar, and then the superficial ones are the high malar and the middle malar. And depending on the facial structure and amount of fat loss, we place fat in different amounts. The access point is the alar base. This is injected deep and it's never over-injected. In this case, we're doing two cc's. And it's injected deep. We will do the superficial fat compartment to follow. Then there is a medial deep fat compartment and a lateral. The lateral deep is the one we're injecting today. That's 45 degrees and it goes right above the periosteum. 45 degrees from the vertical. And that's injected deep in this area and placed in a circular manner in this area. And I will be placing two cc's of fat in this area. It is cardinal that this be filled properly because this blends the eyelid-cheek junction. And that's what we're doing. This is all below the mimetic muscles. The next one is the high malar. And in a case like this patient, where she actually is, this is her narrow side, so we will definitely be placing more in this area. And this is in the SMAS layer. So this will be lifted and we'll be placing fat high in the malar compartment. So we'll be placing that in that area. And then we will transition to the middle malar compartment as we go inferiorly, again, in the mimetic layers. And that will help blend the lateral cheek when you're doing your lift and fill facelift. So we will be placing fat in this area as well. We will be doing the same on the other side.
