A 39-year-old woman with three prior breast surgeries underwent tertiary augmentation mastopexy for severe asymmetry, right-sided bottoming out, inframammary fold displacement, and skin laxity. Exam and measurements showed a longer lower pole on the right, confirming fold malposition and inferior pole excess. Surgery began through the prior inverted-T scar with implant removal and pocket assessment. Because the capsule was healthy, the pocket was converted from subglandular to submuscular using a composite reverse inferior muscular sling (CRIMS), creating stronger lower-pole support and better control of implant position. New fold height and pocket dimensions were planned using sizers to restore symmetry.
