This case demonstrates revision breast augmentation in a 43-year-old woman with severe asymmetry, left-sided bottoming out, and right Baker III–IV capsular contracture after two prior augmentations. Preoperative markings and measurements guided correction of nipple-to-IMF discrepancies and lower pole descent. Through the prior inframammary incision, the implant was removed, the pocket evaluated, and selective capsulotomy/capsulectomy performed while preserving capsular flaps for new fold support. A lateral release with fascia preservation helped prevent lateral displacement. Sizers guided pocket adjustment, followed by definitive implant placement using a no-touch technique. Right-side support improved; left-sided fold correction continued in part 2.
