The septal dissection begins by carefully separating the lower cartilage using sharp scissors, entering the correct plane on the cleft side for better access. The anterior nasal spine (ANS) is identified, and the anterior caudal cartilage point is freed from soft tissue, often displaced on the cleft side in unilateral cleft lips. The non-cleft side dissection is more challenging. The septum is then gently detached from the maxillary crest to release deforming forces, allowing passive straightening and repositioning toward the cleft side of the ANS. Subperiosteal dissection on the cleft side creates space for the septum relocation.
