So what is the best evidence for managing breast implant biofilms? First it's important to note that higher quality studies are still needed to inform clinical practice. I would say that if an antibiotic irrigation is to be used, the best available in vivo and in vitro evidence supports the use of povidone iodine. In cosmetic surgery, evidence supports the use of pre-operative antibiotics only. In reconstruction, the evidence supports at least 24 to 48 hours of post-operative antibiotics. And the minimization of skin and glandular contact and therefore potential for planktonic bacteria to touch the implant by minimizing with funnels or other barriers is strongly recommended.
