Video Summary
A 57-year-old female underwent reconstruction surgery for a perineum and posterior vaginal wall defect following an abdominoperineal resection for recurrent rectal cancer. A vertical gracilis myocutaneous flap was used for the reconstruction. The surgery involved marking and incisions along the thigh, managing blood vessels and nerves, and isolating the flap on its pedicle. Indocyanine green (ICG) angiography was utilized to assess flap perfusion. A tunnel was created, and the flap was transferred to fill the perineal defect. The cutaneous paddle was sutured to the vaginal remnant with the remaining flap filling pelvic dead space. The defect was then closed in layers.
