Video Transcription
So we are here with complex revision rhinoplasty using MTF cartilage. Now you can see we have a nice piece of MTF cartilage, slightly warped because it has been thawed for an hour. We are going to cut this in a way that it will not be warped and utilize it in a way that it won't be warped. This piece is great here. I'm going to start by carving out a nice, thick, rigid piece for our collumellar strut graft here. The way I'm going to use the MTF today for this particular patient is we're going to have a nice piece for the collumellar strut graft because she really needs tip support. That's what this piece here is going to be for. Then we're going to split this piece down saggily like this to use this for two alar contour grafts. Now if I was also using spreader grafts, we would fashion it a little differently. But this patient today needs tip support.
Video Summary
In this procedure, the surgeon performs a complex revision rhinoplasty using MTF cartilage. The cartilage, slightly warped due to thawing, is being prepared for specific grafts. A thick, rigid piece is carved for a collumellar strut graft to provide the patient with the necessary tip support. The remaining cartilage is split sagitally for two alar contour grafts. While the process would differ if spreader grafts were needed, the focus for this patient is on enhancing tip support with these precise adjustments.
