So we're here in the operating room. We've done a lot of great maneuvers for this patient so far, but what I want to demonstrate now is placement of an ALAR contour graft. I place ALAR contour grafts in the nose in order to help prevent ALAR retraction. I'll place a regular ALAR contour graft. If the patient already has severe ALAR retraction, I'll do an extended ALAR contour graft all the way up to the collumellar strut graft. What I'm going to do here is we're going to place an ALAR contour graft right in a subcutaneous pocket of the ALAR right here. It's going to help prevent external valve collapse. It's going to do two things. It helps prevent ALAR retraction and also gives the external valve a lot of nice support. What you want to do, though, is you want to get in here and sort of trim this just ever so slightly just so that it can't be seen through the skin. I cut that little piece there and we'll just remove that. These are great grafts and they can really help to really make a nice-looking nose.
