Care and specialist expertise are concentrated in major cities, while regional hospitals may lack trained microsurgeons, equipment, operating-room capacity or funding for implants and other supplies. Public-sector waiting times and gaps in multidisciplinary coordination can further delay care. In the Philippines, many patients also face substantial out-of-pocket costs and may not be told about reconstruction; awareness and access are generally stronger in Italy, though regional differences persist.<br /><br />Speakers described choosing techniques according to patients’ health, anatomy, cancer treatment, preferences and available resources. Implant-based reconstruction—often immediate and prepectoral—is common, partly because it is faster and less demanding than autologous surgery. Autologous reconstruction can offer durable, natural results but requires suitable donor tissue and specialized expertise. The panel emphasized that decisions should be patient-centered, not driven solely by surgeons’ preferences. When asked what they would choose themselves, several speakers favored immediate implants for speed and waking with a breast mound.<br /><br />Initiatives include breast reconstruction awareness campaigns in the Philippines and public education efforts in Italy. The speakers called for better patient information, multidisciplinary pathways, broader geographic access and stronger public coverage. They also highlighted potential contributions from microsurgery, regenerative medicine, hybrid techniques and technology, while stressing that equitable organization and empathetic care are essential.
