This presentation demonstrates interleukin-2 injection for in-transit melanoma. This patient presented with in-transit metastasis after prior melanoma excision on his left ear. He was started on IL-2 injections. After two sets of injections two weeks apart, his lesions are responding. After a total of four bi-weekly injections, he completely resolved his in-transit disease. This patient previously had a thick melanoma on her calf. Later, she developed multiple in-transits in the left upper thigh and inguinal region. The injection technique is demonstrated here. The injection sites are cleansed with alcohol. We utilize one milliliter tuberculin syringes with lure-lock tips and 30-gauge needles. The IL-2 is reconstituted as per the instructions, and a maximum of 2.4 milliliters is divided into equal aliquots and injected into the palpable lesions. The patient is treated bi-weekly for a total of four sets of injections. Treatment is generally well tolerated. Fever and chills are expected the first evening after injection, and these are easily managed with Tylenol and oral hydration at home. This patient had in-transits along a prior melanoma white excision scar near the elbow and had complete resolution of disease after IL-2. Fortunately, for patients who experience a complete response, the response tends to be durable.
