Video Summary
The first step in the preparation of the recipient site is the elevation of the superior flap. Dissection is directly down to the pectoralis major muscle, very similar to the preoperative markings. The lower limit of the flap is taken down to approximately 1 cm above the inframammary fold. The medial border of the pocket extends to the sternal border and laterally to the full width of the breast. The beginning of the tunnel for the tram flap is started medially in the inframammary fold, again at the subcutaneous level, taking great care not to dive deep to the fascia.
